Abstract

The medical home, palliative care, and hospice settings provide nurses with excellent venues for improving patient care using evidence-based nursing innovations. Nurses can be creative in designing programs that work for their specific populations, regardless of patient age, care setting, or health status. Nurses can partner with the surrounding community to find unique solutions that address social determinants of health by creating transportation programs, helping patients connect with educational and job training programs, finding housing, supporting recreational programs, or even creating a community garden. This chapter includes an overview of medical home, hospice, and palliative care settings. It begins with an in-depth review of medical homes, including historical perspectives, standards of care, and the role of the nurse. Following this, the chapter provides an overview of both hospice and palliative care settings and describes parallel topic areas.
