19: Patterns of Locomotor Recovery After Stroke

Abstract

This chapter describes the patterns of locomotor recovery in persons who have sustained a cerebral stroke. It reviews the recovery trajectories after stroke obtained from longitudinal studies using clinical measures and underlying mechanisms associated with the recovery of strokes of different severities. The chapter presents the data describing the recovery, from stroke onset to two years after stroke, of walking speed and its relation to movements and muscle activations of the lower extremity in a cohort of subjects with a middle cerebral artery ischemic stroke who participated in a clinical trial. It also describes the compensatory kinetic strategies during gait of persons with different stroke severities. Prediction models that are adjusted for the effects of time after stroke suggest that outcome is largely defined within the first weeks after stroke, although functional improvement has been found to extend beyond six months after stroke.
