This chapter provides education templates for input into electronic medical records. These templates cover definition, transmission, signs and symptoms, diagnosis, treatment, contradictions, and follow-up for common conditions. The chapter covers common conditions such as bacterial vaginosis, candidiasis (monilia) yeast infection, care after abortion, cervical cap (femcap), chlamydia trachomatis, contraceptive implant, contraceptive patch, cystitis contraceptive vaginal ring (UTI), diaphragm, genital herpes simplex, genital warts (condylomata acuminata), gonorrhea, hormone therapy, intrauterine device, lice (pediculosis), osteoporosis, polycystic ovary syndrome, preconception care, premenstrual syndrome, scabies, smoking cessation, stress or urge urinary incontinence, syphilis, trichomoniasis, and vaginal contraceptive sponge. The templates have been revised with language that is appropriate for the lay public. They can be edited and customized to specific practice settings and individualized for the women nurse practitioners care for.
