Abstract

This chapter outlines foundational work in the field of implementation science, initial steps for site planning, important guidelines for site leadership, and an innovative use of telemedicine. The Centering Healthcare Institute has developed a plan to assist systems to be successful in navigating each step of the process. Perhaps the most difficult concept is that the Centering model is an alternative to individual/traditional care and not another educational or support program. Identifying the institutional policy changes, recruitment processes, key staff, and protocols takes considerable consensus building by members of the steering committee. Telehealth is used for primary care and specialist referrals as well as for patient monitoring, or for communicating with patients over the Internet. Consumers search daily for information on drugs, diseases, and pregnancy health data. Connecting with their health care providers on a dedicated portal makes getting this information easier.
